The peace of their community is shattered by a violent event, driving Ellie on a relentless quest for justice. The narrative offers players both Ellie’s and Abby’s perspectives, allowing them to experience the events from multiple angles. Core gameplay combines exploration, resource gathering, stealth, and combat, as in the previous title of the series. Players fight against infected creatures and hostile human groups struggling to survive. The Remastered edition does not alter the fundamental story or gameplay structure.

The most significant addition in The Last of Us Part II Remastered is the roguelike survival mode titled "No Return." This mode offers an experience independent of the main story. Players select from a variety of characters, each with unique gameplay traits that unlock as the game progresses. In each playthrough, they attempt to survive a series of randomly generated combat scenarios set in locations drawn from the main game. Rounds feature different objectives such as "Attack," "Hunt," "Capture," and "Defend," and conclude with a boss battle. Due to the roguelike structure, death in a run results in the loss of progress and temporary upgrades acquired during that session. However, completed missions and achievements permanently unlock new characters and starting equipment.

In addition to the "No Return" mode, this edition includes several pieces of extra content and technical enhancements. Under the title "Lost Levels," three previously cut sections from the early development phase of the original game are now playable, accompanied by developer commentary. Another new feature is the "Freeform Guitar Playing" mode, which expands the in-game guitar mechanics. The PC version supports upscaling technologies such as NVIDIA DLSS 3, AMD FSR 3.1, and Intel XeSS. It also includes features such as ultra-wide screen support, HDR, DirectStorage, and full DualSense controller integration via wired connection (haptic feedback and adaptive triggers). The game also offers unlockable new character and weapon appearances for both Ellie and Abby.
